Please plan on arriving 20 minutes before your appointment to allow time for parking and additional paperwork if needed. Neurological consultations and new patient evaluations range in price, depending on the complexity of the examinations, diagnosis, treatment options, and time involved. All co-pays and deductibles are due at the time of your appointment. Please note that we accept assignment only for Medicare, Medicaid, Workers’ Compensation, and some HMOs. (You can contact us to verify coverage of your plan.)
If your injury is related to an auto or work accident, your auto or workers’ compensation adjusters must make your appointment and furnish our office with all the related billing and claim information, along with a prior approval in some cases. If you have any concerns about this process, please contact your adjuster or our office.
Please remember to bring your insurance cards and driver’s license for us to copy, along with your co-pay or deductible if any apply. Please do not hesitate to call our office if you have any further questions.
